Digital COVID-19 Vaccine Record

Dear SCESD Staff, 

COVID-19 UPDATES

Per Executive Order from Governor Gavin Newsom, all school district staff shall be either fully vaccinated against COVID-19 or be tested weekly for COVID-19.  
This Executive Order takes effect as of October 15, 2021.

Please use the following link to answer a few short questions about your vaccination status.

• If you are fully vaccinated, you will have to upload proof of that vaccination status.  If you have misplaced your vaccination card, you may use this link to find proof of your vaccination status myvaccinerecord.cdph.ca.gov.

• If you choose to decline to state your vaccination status, you will follow the same testing requirements as those staff who are not vaccinated If you are not fully vaccinated by October 15, you must submit to testing for COVID-19 on a weekly basis.  

• Testing will take place on each school campus or work site weekly.  School site testing will be scheduled on either Tuesday or Thursday.  For those who will be tested, more information will follow your answers to the COVID-19 vaccination status questionnaire

• If a staff member chooses not to be tested on the school site, they may choose to provide weekly proof of testing from another provider
